I know that if you plant a bunch of saplings or bushes at once, some of them will die even though you didn't exceed 12 in a row. I remember seeing a guide about this on tumblr one time but I can't find it.

I'm trying to make a hedge maze, which has been hard enough with the 12-bush rule, but my bushes are dying for no apparent reason.
 
I have the same problem with my pine trees. They aren't near a rock, PWP, cliff, or body of water, and they aren't in a row. They have plenty of space but they keep on dying anyways. :/ It is a mystery to me.

Pine trees can only grow in the northern half of the town.
 
Oh, I am glad I found this thread before landscapping! I found a guide for making your own ideal town, but it was just a guide on what to CONSIDER when you go to make it (Like deciding on a theme, nothing in depth) So having a guide will help me when I DO go to landscape. I plan on using lots of bushes because my town will be forest themed. I'll be sure to bookmark that guide for when I need it. (Hopefully soon, have almost all my dreamies.)
 
They can't be near anything. Not rocks, ledges, PWPs, walls, houses anything. Also if you plant trees within the chain they count too.
 
You could try occasionally using bamboo stumps? Bushes will grow right next to them with no spaces, and you can't sit on them and hop off outside your maze like you could with a regular tree stump.

Sometimes there are muddy patches that nothing will grow on, including grass! There's no way to fix it, just work around it, they're usually small. I have one right in the middle of my orchard. But if it's a very large space, (bigger than 3x3, probably) then I have no idea!
 
There's another rule that I just discovered today as well. Bushes won't grow if there are more than 15 Trees/Bushes within a 7x7 square (3 tiles in each direction) around the spot in question. I'm not sure if Trees won't grow by that rule though. I have some images that might help:

If I wanted to plant a Bush at the red square it would die because there are more than 15 Bushes/Tress in the 7x7 square.

Oh, and the link Ettienne provided explains Tree-growing pretty well... I'm not sure how correct the information on Bushes is though.
