I had this item on the Gamecube version. Can't remember for the life of me where it comes from, but it TERRIFIED me when I first clicked it. Hah!

As the others said, it's the Italy item from Gulliver.

I got it and decided to check it out.

I had about five hundred heart attacks and started crying. Promptly vendored it after scaring the **** out of my friend with it. What a terrifying item.
