Do I have to spend my heart crystals right away?

Try to spend them because you are limited on buying 8 items MAX a day. Of course some items are expensive, so you might want to save up some for the other days (like the pipe organ that costs 40 crystals). So buy the cheap ones as soon as you can.
If you want to buy at least one of each furniture, you need around 220+ I think. There are around 26 items (some are given), so you can buy all of them within a week. So don't stress too much though since you have a whole month to buy them.

No, you can save them. But I've seen reports that you can only buy up to 8 items per day from Cyrus with them. It doesn't matter now, but it will at the end of the month.
I personally am going to spend them as I go and then just save up my heart crystals when I can't buy anymore that day. So today I bought a wedding decoration and a blue wedding rug, and I'm going to save up for the other items in future days.

Is there any way we can find out which items are given to us? I don’t want to purchase something and then get it for free
There’s certain items you unlock for the first week. You’re given a new item from the patch that gets unlock each day, so don’t worry about having order something today and getting it as a reward tomorrow.
It varies. Depends ultimately how well you did. For the first week you can get a max of 11 crystal hearts each day.
