So I am not clear on what exactly counts as trash on the ground in order to remove it and receive the perfect town status.

I know baskets of fruit and items, but does anyone know if lost items and things like mushrooms count as well?

mushrooms dont count but any other item on the ground counts

Thank you so much! I was worried picking up mushrooms and placing them somewhere else would make them count as trash.

Yeah, I used gems on the ground as part of my towns decoration and promptly lost my perfect town rating because they were considered trash. But clovers and mush don't count as trash

What about seashells? Even though I'm sure I've never had a town close to perfect, I've always put as many seashells as I could fit on my bridges and ramps down to the beach.

Seashells are safe, I have a lot in my main town and have a perfect rating. :)

Pretty much anything that spawns in your town naturally isn't trash. Seashells wash ashore, fruit falls from trees, mushrooms spawn next to special stumps or in November, clovers spawn naturally. Weeds are trash, however, and so are lost items (the villagers dropped them there, not nature!).

Are you sure about that? For the past few weeks my villagers have been complaining about the cleanliness of my town. Understandable, since I had to dump my entire closet onto the beach because of storage needs for turnips. Now, however, I've picked everything up - except for a few 4-leaf clovers, and they're still complaining. Does it take a few days for the villagers to notice that I cleaned everything up, or do clovers indeed count as trash?

No, clovers don't count as trash. They're barely items, actually. They might complain because there are too little PWPs, trees or flowers. There also 'good' and 'bad' PWPs, some might be called trash by the villagers?
Are you sure you don't have any fruit baskets? Although they do come from the 'natural product' fruit, they have been assembled by a person into a basket which makes them unnatural for that matter. By the way, rotten fruit (from dead perfect fruit trees) is trash, as well as (rotten) turnips.
