It's called QR MyDesign. If you search animal crossing in the Apple App Store it should be in the listings. It's specifically for Animal Crossing and you can make patterns and faceboards with pictures on your iPod/iPhone. You are even able to change the name and town info, as well as the title of the new pattern within the app.
