Hey! I did a search by image, and found that its from this tumblr: http://www.sosostris.com/ unfortunately, she hasnt posted the code for it, but it looks like she takes requests for qr codes, and im sure if you sent a quick message asking to post the code she would! sorry i couldn't be any more help :/ I also recommend looking at pathmagic.tumblr, acnlpaths.tumblr, and acpaths.tumblr, they have almost every path code!
