Personally, I think jack is a spirit possessing a pumpkin head, and the pumpkin is his actual head.

Jack is clearly a ghost or spirit of sorts from a time very long ago, back when Czars were a thing. He loved Trick or Treating so much that when he passed on he took possession of a Jack o Lantern to keep the spook alive.

I think Jack is just Jack, a guy with a pumpkin head. It would be too weird for him to be someone in disguise, but I do admit, some of the theories others here have said were pretty good.
