I'm currently drawing a blank when it comes to picking themes for my alt characters' houses and could use some help coming up with something. I thought of making one house into a hospital or lab but I'm not sure if there are enough furniture items in the game to do that... I'm still not sure about my town theme itself (leaning towards a rural town or a modern town - opposite ideas, I know) so it's not helping me decide :\ Does anyone have any suggestions before I make the characters?

You could definitely make a hotel work for a modern town or a rural town. (Big fancy hotel vs. quaint bed and breakfast type place) That could be cool. 
Twisk (Amy) made a really beautiful tavern in her town, and you could do something like that. I think it could work well with a rural theme. Here's some pictures for inspiration:

I made my alt. house into an ocean themed hotel.
I think hotels are great options for either a modern or rural town. Out of curiosity, what is the object that is being displayed on the table (the drink thing)?

Thank you! I didn't know you can display the harvest festival items!
With the harvest festival coming along, maybe you could also make a restaurant too for an alt. house! 
A hospital would work you could make the main room the reception desk and waiting area. And, have the other rooms as the doctors office, a lab, and patient rooms, perhaps even a cafeteria in the basement.
A hotel as others recommended also works great and fits any theme.
Perhaps for your third alternate a shop with each room being a different department. Another option is a library. The main room a check out desk and sitting area and the other rooms, the main book stacks, a reading room with tables, a kids reading room, and a room with public computers and copy machines.

I have a hospital in my town with pretty similar exactly the same room designs (waiting room, office, a room with hospital beds), but I included a dentist room as well.

Put a Zen bell outside and have bamboo around the house! Doesnt need to be full upgraded but can have lucky cats, incese burners and samurai suits and leave bells around so it looks like people made a wishes there. Have Merlions in the back, along with a full moon vanity and bamboo grass (Idea by Nammmie). Maybe have a Bonsai room upstairs or to the side.

Use Hot plates, Flour, Sugar, Butter, Flour, Milk, Vinager, Turkey, Sushi platters, Tea set, Afternoon tea set and Stewpot, they are some of the items that look best! The back room can be the kitchen while the left and right rooms can be private rooms or rest rooms for people. You can also make different kind of restaurants like breakfast (hot plates and coffee cups), fancy dinners (turkey and afternoon tea set), japanese (Tteok plate and kimbap plate) and even a Diner (diner tile and stools).

Get items from the Seven Eleven set, Sandwich display, Soft-drink display, Counter and Buffet Survers look good. Or you a Checkout counter and Cash register instead! Modern Wood furniture looks best in my opinion. Put Jugs of Milk, Vinegar, Butter, Flour and Sugar on tables and have Shopping carts on the sidelines to be used. Milk cases and Ice cream cases look nice beside the register.

Only need the First floor rooms, with the Boy side on the left and Girls on the left, Have Wooden or Original Bathtubs along with Washbasins and buckets, and wooden stools people can sit on. Hospital screens look good if you use the Bathhouse tile and Bathhouse wall. Bathrobes and Bath mat are good items, and maybe the Aroma pot. Boys can wear Boxing shorts as boxers while girls can wear the Bathrobes and Headbands. The backroom can be seperated by Screens and have shoes and clothes on the ground. First room can have circle banners along with Milk cases, checkout counter and cash register and also have cubbyholes with shoes placed on the ground.
→ Karaoke
Can have screens to separated the room in two, have kotatsu in the middle, tatami flooring if you want it japanese like. Have Snacks like New year noodles, Kimbap plates, Ceramic hot pot and tteok plate. Cushions can be customized on the floor. Have the Karaoke system along with a mic stand upfront for the preformer. Glass top tables look nice, place fruits ontop of them, it looks nice.

You can have a Electronic area for TVs (flat screen, retro, karaoke system,Modern wood, TV with VCR, Wide screen, etc), Kitchen area (Fridges, Counters, Kitchen Island, Stoves, etc), Maybe have a separate area from small appliances like Toasters, Microwaves, Blenders, etc. in different colours, have cardboard boxes that looks like it has the appliance in them! Maybe have a Mattress area with different beds and cushions on tables.

Holiday house. Good place to display all the holiday. Front room has the current holiday items on display.

Maybe an auction house? Set up some rare items in there ready to be "auctioned". A schoolhouse would work.
I made one of mine a bath house(but I still can't get the bathtub and other stuff I need for it, since they never show up in the TIY). It has an upstairs with tatami beds for resting after bathing, and a bath room of course. I think I will add a dining room to it. It'll be a little bath and lunch place. It has bamboo and a zen garden out front, too.
I can't remember where I saw it, but I remember a house made to look like a petstore. It had bags to look like pet food, doghouses, hamster cages, fish, and other stuff.

Lost in LalaLand...
Before I planned LalaLand out to be what it is now, one of my alt characters' homes was basically a big store. I had the main room decked out with the 7/11 series, I had two sets and used most of the pieces, using Pachiras as fillers for corners and a sleek stereo playing KK Disco (Idk why that makes me think of stores but it does) in the corner. One of the side rooms was a candy shop, another was a coffee shop - I can't remember what I did with the back room or the basement, although it seemed like I used the basement for "storage" for the coffee shop with some cardboard boxes here and there with some coffeebags scattered around. And the upstairs was the office. I had a computer stand for each shop, a copier machine, a water cooler, a time clock.
