Take to some place that can take the screw out, patch it, reseal it, and re-inflate it. ((some oil change places to tires, too, but call ahead to make sure))
I don't know any DIY jobs for that problem, though.
Is it like a really sharp screw? Take the wheel out after it deflates. Just replace the wheel after. You won't be able to do anything unless you quickly take the screw out and patch the whole with duct tape or something.
You won't be able to put it out yourself most likely. And if you do, a big hole will be left. I'd say leave it in & let a mechanic take a look. You wouldn't want the tire to explode when you're picking at it.
