Not usually. Sometimes the old ones at hotels that shake and stuff freak me out a little but usually I'm fine as long as I'm not by myself.

yes. i instinctively jump whenever i spot one of those in my peripheral vision. is it a spider? a beetle? just a fly?? that second of uncertainty is what makes it scary.
how about dark, murky bodies of water? swimming in lakes (and sometimes the ocean) gives me the creeps when you have no idea how deep it is and what lurks beneath.
Yes actually. I really hate that. I'm a strong swimmer, but the idea of not being able to see what's lurking creeps me right out.
Churches?

I've never encountered one in real life, so for the time being, I'm not scared of them (yet). I actually think they are cool animals.

Depends what kind... but I'd probably say yes.

No, cos I live in the uk and therefore get mostly free healthcare. So I would assume it was just a con and throw it away.

No. Why would they? I think they are pretty and useful! Feathers to fly, feathers to write (as quills), feathers for a soft pillow or warm comforter... and of course, beautiful feathers that some birds have.
That creepy old porcelain doll that your granny used to have on the shelf? Or a similar situation, you know grannies...
I didn't really get to know any of my grandparents well, nor did they have porcelain dolls that I can recall, but even if they did I'd have to say no.
