I don't think the QR code is anywhere on the web; I found the tumblr blog of the person with those tiles in his/her town but the qr code for this tile isn't on it. I'll have a look at the dream address though, perhaps you can get it there. (The picture above belongs to this post)

Wendell didn't carry the pattern... Perhaps you could ask the owner of the tumblr blog if she has the QR code?
I wouldn't recommend visiting the dream by the way, it's hacked all over and you constantly get suck between rows of trees and bushes. >.<
