Early on in my playthrough, I was looking for Gala to give her a present. I checked EVERYWHERE. In the end I found her when I was walking along the cliff and saw her on my secret beach. As this was early on, I didn't have a wetsuit and hadn't been to the beach yet. It's totally unaccessable.
It never happened before that and it's never happened again. It was very odd.
