So basically, what is it? I've looked around and it's kind of confusing to me.
Like.. what do you do?

Its basically a forum plus twitteresque features. Instead of a board for I don't know Off-topic discussion or whatever there is a board or community for each game on Wii U and 3DS for the most part.

Almost every game, as it sometimes takes a while for Nintendo to add communities for some (mostly not that popular) games.

Also, just like Twitter and other forms of social media, it's filled up with idiots. Don't try to communicate with them or you'll start losing brain cells.

If you have a Wii U you can just request each other like on any other console but for 3DS nope.

From what I remember they said they were working on integrating such functionality into the 3DS Miiverse...

... back when it came out.
