Hope the reason it didn't appear for me was that my pop-up blocker was up. That would suck.
I'll probably find a .rar or something later on or buy it off of ebay. Who knows, maybe Jasonburrows will sell his to me.

Yeah, I got mine about 30 minutes ago, popped up right after the survey. I had my pop up blocker on too though, but I was in safari on a mac, and I don't know what you were using.

I had the same problem. Apparently a lot of people did. They didn't run out, it's just an error on the site that doesn't give you the pop-up to accept the offer. Call or e-mail Nintendo, they'll fix it for you.
