How do you get them?

By items do you mean collectibles or things like bigger mailboxes? Mailboxes you simply purchase for them to take effect, collectibles just sit there to look pretty. Most everything on the main shop page takes effect as soon as you buy it minus things that require manual input like name changes (I think).
